Part-time Evening MBA Decatur Shuttle Agreement & Payment Form

Filling in your credit card information below and submitting this form indicates agreement to the statements listed below.  A new agreement is required for each module.

A vendor, designated by the MBA program, will provide round-trip van transportation each Monday and Wednesday when classes meet for the dates listed below.  A module is defined by the Part-Time MBA course schedule.  The van will arrive in front of the Von Maur store entrance at the Hickory Point Mall in Forsyth at 4:50 PM and leave promptly at 5:00 PM for the University of Illinois, Business Instructional Facility (515 E. Gregory Drive, Champaign). The van will depart from Business Instructional Facility at 9:15 PM and return the students to the same site. Only Part-time Evening MBA students who have signed this agreement and paid the required fee may ride in the shuttle van.

The cost of van transportation for the dates listed below is $100 (10 weeks/20 trips) and is nonrefundable, whether or not all rides are taken.  The departure time of the van is determined by the MBA program.  The driver will not be able to grant requests to change these times.  The MBA program reserves the right to make adjustments to the pick up and drop off points and times.  The van will leave promptly at the scheduled time and will not wait for students who are not present.  Food, coolers, and beverages are allowed in the vehicle.  Smoking and alcohol will not be permitted at any time. 

Payment is due Wednesday, April 7, 2010.
